Installer Application

You are applying to work as a subcontracted installer for Floor Craft LLC. This is in no way an application to work as a direct employee of Floor Craft LLC. In the event that subcontracted work is pursued, Floor Craft LLC requires a W-9 to be filled out, proof of liability insurance naming Floor Craft as a certificate holder/additional insured, a tax ID number, trade name registered with the state, proof of Workmans Comp Insurance if you have employees and a signed copy of our Independent Contractor Agreement.
